An Arosa Care Manager steps in at that moment to assess your loved one’s physical, emotional, social, and spiritual needs. Then they work with the family and care team to develop a Care Plan that provides the appropriate support.
As your Care Manager gets to know you and your loved one, they gain an understanding of your family’s needs and preferences. This allows them to connect you to resources that could be beneficial for you. They also ensure a smooth start of care, making adjustments as necessary.
Your Care Manager stays involved, overseeing your loved one’s care, supporting and coaching the caregivers, and making scheduled home visits. They also communicate with responsible parties and advocate for the client.
