Jennifer Axelson is the Senior Director of Client Services. She’s a Licensed Clinical Social Worker, Certified Case Manager, Certified Dementia Practitioner, Certified Alzheimer’s Disease and Dementia Care Trainer, Certified Life Care Planner, and a Certified Medical Cost Projection Specialist. With over 13 years of experience as a care manager and clinical supervisor, Jennifer has created, implemented, and/or overseen hundreds of assessments and customized care plans for individuals. These plans have included measures related to aging, functional limitations, personal care services, aging in place, advanced directives, maintaining independence, maximizing quality of life, chronic pain management, mental health, and complex medical issues.

In her free time, Jennifer enjoys spending time outdoors with her husband and two daughters, as well as watching football, playing sports, and enjoying historical documentaries.
