Jennifer Axelson, LCSW, CCM, CDP, CAD-DCT is the Executive Director of Operations at Arosa, the nation’s largest provider of integrated care management and caregiving services.

She is a Licensed Clinical Social Worker, Certified Case Manager, Certified Dementia Practitioner, and Certified Alzheimer’s Disease and Dementia Care Trainer. With over 15 years of experience as a care manager and clinical supervisor, Jennifer has created, implemented, and overseen hundreds of comprehensive assessments and customized care plans.

These care plans have addressed a wide range of needs, including aging and functional limitations, personal care services, aging in place, advance directives, maintaining independence, maximizing quality of life, chronic pain management, mental health concerns, and complex medical conditions.

In her free time, Jennifer enjoys spending time outdoors with her husband and two daughters, as well as watching football, playing sports, and enjoying historical documentaries.
