Sommer shared a story about a client experiencing cognitive decline who had been living alone with some support in place. One night, a safety situation changed everything.

“There was a lot of fear and urgency,” she explains. “My role was not just to coordinate care, but to reassure the family and help them move forward with a clear plan.”

The team quickly put 24-hour care in place and supported the family through next steps. That included having honest conversations about long term care and eventually helping transition their loved one into a safer environment.

“What started as a really overwhelming situation became a turning point toward stability and peace of mind.”

“Supporting families during difficult transitions starts with listening,” Nitce says. “We take the time to understand their concerns and what matters most to them.”
