What Does Care Management Look Like?

Arosa Care Managers work closely with you throughout the care journey to ensure care is delivered in a manner that supports your loved one’s physical, emotional, and social wellbeing.

  • Crisis Intervention

    Families often begin the care journey after an illness, injury, or diagnosis makes it difficult for a loved one to safely care for themself. An Arosa Care Manager steps in at that moment to assess your loved one’s physical, emotional, social, and spiritual needs. Then they work with the family and care team to develop a Care Plan that provides the appropriate support.

  • Care Coordination & Implementation

    As your Care Manager gets to know you and your loved one, they gain an understanding of your family’s needs and preferences. This allows them to connect you to resources that could be beneficial for you. They also ensure a smooth start of care, making adjustments as necessary.

  • Ongoing Support

    Your Care Manager stays involved, overseeing your loved one’s care, supporting and coaching the caregivers, and making scheduled home visits. They also communicate with responsible parties and advocate for the client.

Guardianship / Conservatorship

Expert, Unbiased Management of Your Loved One’s Medical Needs

When your loved one is unable to make or communicate safe decisions about their healthcare, Arosa Care Managers can support them and the family in an official, court-appointed role. We have experience serving in a multitude of capacities, including Guardian of Person, Limited Guardian, Temporary Guardian, and Lifecare Manager.

If your Care Manager is called to act as your loved one’s HCPOA, they may:

  • Consent to treatments
  • Coordinate care
  • Follow desired wishes previously identified as long as safe and affordable
  • Make end of life care decisions
Healthcare Power of Attorney

Supporting Hour Loved One’s Healthcare Wishes

A Health Care Power of Attorney (HCPOA) lets your loved one choose who will speak for them (the Agent) if they’re unable to safely communicate their own healthcare decisions. And it provides the guidance that their Agent must follow. Having a HCPOA in place can reduce family conflict and future court involvement.

An Arosa Care Manager may be named as the Agent or Successor Agent (in the event the Agent can’t fulfill their role when needed) of the HCPOA. As an Agent or Successor Agent, your Care Manager will insure they’re always ready to act by:

  • Visiting with you and your loved one, quarterly to annually as necessary
  • Regularly reviewing medical history and desired end of life wishes
  • Staying up to date on overall needs and health
